Tax Information

Tax forms are now available at the library. See information below for more details.

Wisconsin Forms

The State of Wisconsin is providing all the usual forms and booklets, including the Homestead form (Rent Certificate). You can also download Wisconsin forms and booklets from http://www.revenue.wi.gov/html/formpub.html

Federal Forms

Due to cuts in funding for the Tax Forms Outlet Program (TFOP) libraries will no longer be provided with a full range of federal tax forms for the public. We will only have federal forms 1040, 1040A, and 1040EZ available this year. Tax forms and instruction booklets not available at the library can be obtained in a variety of ways:

  • Download from the IRS website - http://www.irs.gov/Forms-&-Pubs
These can be printed at home or at the library. There is a charge of 10 cents per page (20 cents for a two-sided page) for forms printed at the library.

  • Order online for home delivery – http://www.irs.gov/uac/Forms-and-Publications-by-U.S.-Mail
Search for a form or booklet by name in the box in the center of the screen. Follow instructions on the screen to complete order. Forms arrive in 7-15 days.

  • Order by phone for home delivery - call 1-800-829-3676
